As a Registered Nurse, I am a licensed medical professional with a diverse range of skills including: evaluating patient conditions, care coordination and managing complex patient cases. I offer in-person, Virtual Health and Telehealth services.

(Serving WhiteRock, Surrey, Langley and Cloverdale areas. Virtual/Telehealth Services are offered to anyone in the lower mainland)

 

 

Skilled Nursing Services and Support

With my oversight I will perform in-home health assessments and a wide range of home care services that meet either short-term or ongoing medical needs. Services may include, but are not limited to:

• Acute or Chronic Disease Management (Individualized Care Planning and Case Management)

• Holistic Health Checks and Assessments

• Palliative Care Support

• Medication Management, Administration and Oversight

• Resource Referrals and Management- (Physio, OT, Home Support, Speech Therapy, GP, Dietitian, Social Work, Podiatrist etc.)

• Wound Assessment/Management

• Catheter Care/Management

• Injections

• Ostomy Care/Management

• Alzheimer’s/Dementia Management and Monitoring

• Post- Surgery Care
